Child dental benefits schedule - patient consent forms
Dentists who bulk bill patients under the Child Dental Benefits Schedule (CDBS) must discuss the treatment and costs for basic dental services before providing the service. If the patient, or their parent or guardian, agrees to the services and cost, they must sign this consent form.
Consent forms for both private and dental clinics are available.
